In my case, applying parche 11 for SAP GUI 7.40 solved the missing icons issue while using the blue crystal theme. If that does not fix anything maybe your alternative is to use the corbu or signature theme.
Blue Crystal and Belize Themes are available as of Microsoft Windows 7. For lower Microsoft Windows releases, the values for Blue Crystal and Belize Themes are treated as invalid entries and SAP Signature Theme is used. Enjoy, Tradeshow and Streamline are not supported anymore as of SAP GUI for Windows 7.60. SAP Blue Crystal is the new visual design theme, introduced by SAP Fiori, which succeeds the older Corbu design. It is based on the same interaction design and UI controls as Corbu, but uses different colors. This new visual design offers a modern and fresh look. In the past with older SAP themes it was possible to adjust the color / hue of the theme for the SAP GUI to differentiate easily between different / critical systems (e.g. Displaying the production environment in red). As far as I can tell, this is not possible with the latest themes (neither Blue Crystal nor Corbu).
